Appointments

Appointments can be made by calling our office Monday thru Friday 9am to 5pm.   We will make every attempt to schedule urgent appointments within 48 hours. Hearing and allergy testing will be scheduled after a physician evaluation.

Patient Forms / Portal

Our office strives to maximize your time with our physician and minimize your time doing paperwork. We provide an on-line “patient portal” to complete check-in paper work. It’s quick and easy! We strongly encourage you to complete these forms online prior to your visit. Here are the steps involved.

  1. You will receive an email from Stephen White ENT inviting you to fill out the paperwork. VERY IMPORTANT: this email often lands in the spam folder, so check that folder!
  2. You will be asked to enter a password. Simply type the word password (all lowercase) you will be asked to enter your pin. This is your birth year.

We strongly encourage you to fill out the patient portal forms. If for some reason you can not complete the online forms, then you must come 20 minutes prior to your scheduled appointment time.

No Show Appointments

We are committed to providing you an appointment at your earliest convenience; likewise, we request a courtesy call at least 24 hours in advance if you are unable to keep your appointment to allow for other patients to be seen.  Multiple missed appointments may result in fees or request for you to find another specialist.

Allergy testing requires significant preparation prior to your appointment. All appointments for allergy testing must be canceled by 3 pm the day before the scheduled appointment to avoid a $50.00 no-show fee.

After Hours and Emergency Care

If you are experiencing an emergency dial 911 or go to the nearest emergency room. For urgent calls an on-call physician is available 24 hours, 7-days a week.  We are happy to assist you, but please limit calls to urgent matters only.

Prescription Renewals

Prescriptions are renewed during normal business hours. If refills are present on your prescription please call the pharmacy first as they will be able to issue the medication.  We may require that a patient be seen in our office prior to medication renewal.

Medical Records/Forms

Medical records or billing records must be accompanied by a signed request and authorization for release of information. The records will be provided within 10 days of request. Forms to be completed including FLMA and disability paperwork will be completed in a timely manner. A fee of $35.00 may be assessed depending on the time necessary to review records and complete the required paperwork.
